Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing environment
• Administer medications and treatments accurately and safely
• Perform thorough assessments and monitor patient status, reporting any changes promptly
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Document care and clinical observations clearly and timely in the patient record
• Support residents and families with education, reassurance, and discharge planning as needed
• Assist with wound care, ADLs and other nursing procedures per facility standards
• Participate in shift handoffs and communicate concerns to ensure continuity of care

Qualifications

• Active Registered Nurse (RN) license — state licensure required
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Comfortable working in a skilled nursing/long-term care environment
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork, and time-management abilities
• Immunizations and required health screenings up to date per facility policy

Education

• Associate Degree in Nursing (ADN) required; BSN preferred

Knowledge & Skills

• Clinical assessment and monitoring of adult and geriatric patients
• Medication administration and accurate charting
• Wound care and basic nursing procedures common in skilled nursing
• Electronic medical record (EMR) documentation skills
• Ability to prioritize tasks and manage multiple patients safely
• Strong interpersonal skills for collaborating with residents, families, and staff
