Roles & Responsibilities

• Coordinate and lead discharge planning and care coordination for complex inpatient patients to ensure safe transitions
• Manage an active caseload (approximately 15–20 patients) with timely chart review and screening
• Document clear, thorough progress notes and case plans in Cerner to support continuity of care
• Collaborate actively with the multidisciplinary team — physicians, respiratory, pharmacy, social work and others — to remove barriers to discharge
• Perform utilization review and resource coordination to match patient needs with appropriate services
• Prioritize tasks effectively in a fast-paced acute care setting to keep patient flow moving
• Facilitate home oxygen referrals and other post-discharge arrangements with partner departments

Qualifications

• Active Registered Nurse (RN) license in Washington (required)
• BLS certification (required)
• Minimum 2 years of nursing experience, with acute inpatient or case management experience preferred
• Comfortable using Cerner EMR for charting and care coordination
• Strong organizational skills, ability to multitask and prioritize in a fast-paced environment
• Excellent written communication for thorough, informative progress notes
• Certifications like CCM, ACM, C-SWCM, or CBHCM are a plus but not required

Education

• Active RN license (ADN or BSN acceptable) — BSN not required
• BLS certification (required)
• Specialty certifications (CCM/ACM/C-SWCM/CBHCM) encouraged if available

Knowledge & Skills

• Expertise in discharge planning and inpatient case management workflows
• Proficiency with Cerner electronic medical record system
• Understanding of utilization review principles and multidisciplinary team dynamics
• Solid clinical judgment for complex medical patients across CCU, step-down, med-surg and neuro
• Ability to coordinate referrals (including home O2) and work with respiratory/pharmacy supports
• Strong prioritization, multitasking and time-management skills
