Location: Carmichael, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM, with rotating weekends (at least one full weekend per month)

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct initial assessments and discharge planning for a caseload of 15-20 patients per day
Perform utilization review using Interqual criteria and coordinate care with all payers including uninsured and homeless populations
Collaborate with interdisciplinary team members for effective discharge planning
Utilize Cerner EMR for documentation and care coordination
Reprioritize daily tasks to meet patient needs and department demands

Qualifications

Current RN license in California
BLS certification required
Experience with Interqual and Cerner EMR
Prior recent experience with Dignity Health (CommonSpirit) preferred
Two supervisory references required (one within 12 months, one within 3 years)

Knowledge & Skills

Case Management and Utilization Review
Discharge Planning and Care Coordination
Dealing with all payers including uninsured
Homeless care experience
Strong collaboration, teamwork, and communication skills
Ability to reprioritize throughout the day
