Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients across greater Sacramento county in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Deliver personalized care plans that help patients regain strength, mobility, and independence
Collaborate with interdisciplinary teams to ensure seamless continuity of care
Document patient progress accurately and communicate updates with the care team
Manage a productive schedule of 5-6 patient visits per day while maintaining quality outcomes

Qualifications

Active Physical Therapy Assistant license in California
Minimum experience requirements vary, new grads considered in exceptional cases
Strong clinical skills across multiple patient populations
Reliable transportation and willingness to travel within the service area
Excellent communication and time-management abilities

Knowledge & Skills

Home health care experience preferred
Ability to work independently in various home settings
Familiarity with diverse diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Strong documentation and organizational skills
Comfortable using technology for scheduling and patient records
