Roles & Responsibilities

Provide compassionate physical therapy care to patients in their homes throughout greater Sacramento county
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain mobility and independence with personalized treatment plans
Collaborate with interdisciplinary teams to deliver exceptional patient outcomes
Document patient progress and maintain accurate clinical records
Build meaningful relationships while making a lasting impact in the community

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New graduates considered in exceptional circumstances only
Strong communication and interpersonal skills
Valid driver's license and reliable transportation

Knowledge & Skills

Experience with diverse patient populations including cardio, pulmonary, orthopedic, neurological, and geriatric
Ability to work independently in home health settings
Strong organizational and time management abilities
Excellent documentation and clinical reasoning skills
Compassionate approach to patient-centered care
