Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility
• Assess patient function, develop individualized treatment plans, and adjust care as needed
• Document patient progress accurately and timely in accordance with facility policy
• Collaborate with nurses, aides, and interdisciplinary team members to optimize resident outcomes
• Meet productivity expectations (therapist productivity requirement: 90%)
• Follow Continuum Rehabilitation Group policies and procedures to ensure quality and compliance

Qualifications

• Licensed Physical Therapist (PT) — DPT credentials listed as a primary skill
• 1-3 years of clinical experience (as indicated in the job posting)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Ability to be on-site during scheduled shifts to receive guaranteed hours
• Willingness to adhere to CRG policies, dress code (black scrubs), and productivity standards
