Roles & Responsibilities

• Perform comprehensive patient evaluations to identify functional deficits and set meaningful goals
• Design and implement individualized treatment plans to improve mobility, strength, and independence
• Deliver evidence-based physical therapy interventions and therapeutic modalities
• Educate patients and families on safe movement, home exercise programs, and adaptive strategies
• Collaborate with nurses, occupational therapists, physicians, and the interdisciplinary team to coordinate care
• Monitor patient progress, adjust treatment plans as needed, and document outcomes clearly and promptly
• Ensure all care and documentation comply with facility policies and regulatory requirements
• Advocate for patient-centered care that enhances quality of life and fosters recovery

Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California (or eligibility to obtain one)
• Experience in a skilled nursing facility (SNF) or similar post-acute care setting preferred
• Strong knowledge of therapeutic modalities, adaptive equipment, and mobility training
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work both independently and as part of a multidisciplinary team
• Compassionate, patient-focused approach with strong clinical decision-making

Education

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
