Roles & Responsibilities

Treat patients with movement dysfunction or pain to improve daily function and reduce risk of adverse events such as falls and DVT
Develop and implement therapeutic exercise plans and mobility training to increase independence
Contribute to discharge planning to ensure safe transitions and continued recovery
Document all patient care activities thoroughly, including evaluations, progress notes, discharges, daily notes, and insurance communications
Communicate regularly with other providers, insurance companies, patients, rehab staff, and supervisors to coordinate care
Perform light upkeep of treatment areas including assisting with cleaning and laundry as needed
Perform other duties as requested, stepping in where the team needs you

Qualifications

Licensed Physical Therapist in Wisconsin (required) — bring your state credentials and confidence
Current CPR/BLS certification (preferred)
Comfortable with patient evaluations, pain management techniques, fall risk reduction, and mobility training
Strong clinical documentation skills and ability to manage insurance communications
Compassionate communicator who can educate patients and families effectively

Education

Doctor of Physical Therapy (DPT) preferred, or Master's in Physical Therapy with appropriate licensure
