Roles & Responsibilities

• Treat patients with movement dysfunction or pain to improve daily function, mobility, and independence.
• Develop and implement individualized treatment plans focused on pain management, fall-risk reduction, and post-surgical recovery.
• Document all patient care activities thoroughly — evaluations, progress notes, daily notes, discharges, and insurance approvals/denials.
• Communicate clearly and professionally with patients, family members, insurers, physicians, rehab staff, and supervisors.
• Assist with discharge planning to support safe transitions and optimal patient outcomes.
• Support clinic needs by helping maintain clean treatment rooms, assisting with laundry, and ensuring a safe therapy environment.
• Perform other duties as requested to contribute to a smooth, patient-centered clinical operation.

Qualifications

• Active or eligible Physical Therapist license for Wisconsin (must be able to obtain state licensure).
• Strong experience with patient evaluation, treatment planning, and documentation.
• Comfort with insurance interactions, including approvals and denials, and clear phone communication with payers and providers.
• Demonstrated ability to manage fall-risk assessment, pain management strategies, and mobility improvement interventions.
• Excellent verbal and written communication skills and a collaborative team approach.
• Ability to perform light clinic maintenance tasks (treatment room cleaning, laundry) as part of daily duties.
• BLS/CPR certification preferred; prior clinical experience in an acute or post-acute setting is a plus.

Education

• Degree in Physical Therapy (DPT preferred) or equivalent degree in physical therapy/related field
