Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's functional status to create tailored care plans
Guide patients and families through therapeutic exercises and equipment use
Collaborate with physicians and care teams to track progress and adjust treatments
Provide education and support to caregivers so recovery continues beyond each visit
Document clinical notes and progress summaries that directly impact patient outcomes

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and family education on therapeutic appliances and exercises
Understanding of Medicare, Medi-Cal, and Title 22 regulations
Ability to evaluate home environments for safety and rehab potential
Clear documentation and timely reporting skills
