Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgeries, falls, and other conditions in their homes
Assess patient needs and create tailored treatment plans that promote independence and mobility
Collaborate with Home Health and Hospice teams to ensure seamless, compassionate care
Document progress using HomeCare/HomeBase EMR and communicate effectively with the care team
Travel within a 60-mile radius of Valley City to bring expert therapy directly to patients who need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related skills and a passion for patient-centered care

Knowledge & Skills

Physical Therapy expertise in home health settings
Comfortable working independently with minimal supervision
Familiarity with HomeCare/HomeBase EMR system
Excellent communication and time-management abilities
Compassionate approach to caring for Home Health and Hospice patients
