Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start of care assessments and develop personalized treatment plans
Perform recertifications and resumption of care visits as needed
Complete approximately 6.5 points per day with a mix of regular visits and admissions
Collaborate with clinical coordinators and directors for support and guidance
Document patient progress using OASIS and maintain accurate clinical records

Qualifications

Current ND Physical Therapy License required
BLS certification required
Valid Driver's License and Auto Insurance required
Home Health experience preferred
OASIS training preferred
First time travelers welcome to apply

Knowledge & Skills

Physical Therapy expertise in home health settings
Strong assessment and care planning abilities
Excellent communication and documentation skills
Ability to work independently while staying connected with the care team
Comfortable with a 50-mile radius travel requirement
