Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day using your clinical expertise
Document care accurately using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to coordinate seamless patient care

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience if they have Home Health background
Strong assessment and documentation skills
Ability to work independently while providing compassionate care

Knowledge & Skills

Physical Therapy expertise in home health settings
Experience with orthopedic and geriatric patient populations
Comfortable managing patients with multiple comorbidities including CHF and COPD
Proficient with EMR documentation and tablet-based workflows
Excellent time management for efficient daily visit scheduling
