Location: Sunnyvale, California

Start Date: 07-13-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible based on patient availability)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Restore function and minimize disability for patients following disease or injury
Help clients achieve their maximum performance and function
Evaluate and treat patients using medically prescribed physical therapy programs
Supervise and guide Physical Therapy Assistants to ensure quality care
Conduct initial admission and evaluation visits to assess patient needs
Collaborate with the Clinical Team Manager by reporting OASIS scores in a timely manner
Document clinical findings, interventions, goals, and discharge plans to ensure continuity of care

Qualifications

One year of Home Health experience required
Must hold an active Physical Therapy license in California
BLS/BCLS certification required
Ability to work independently and manage a caseload
Strong communication and documentation skills

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Skilled in physical therapy evaluation and treatment techniques
Ability to supervise support staff
Excellent time management and organizational skills
Compassionate and patient-centered approach
