Contract Type: Per-Diem

Shift Type: 2nd shift, 3:00 PM - 11:30 PM (with 30-minute meal break; per diem, days may vary)

Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Provide compassionate direct patient care to residents
Administer medications and treatments as prescribed
Monitor vital signs and report changes to the healthcare team
Assist with daily living activities and promote resident independence
Collaborate with nurses and staff to ensure quality care
Maintain accurate documentation and communicate effectively with families

Qualifications

Valid Licensed Practical Nurse (LPN) license in Pennsylvania
Current CPR certification (BLS)
Previous long-term care or skilled nursing experience preferred
Strong communication and interpersonal skills

Knowledge & Skills

Proficient in medication administration and clinical procedures
Knowledge of state and federal regulations for nursing care
Ability to work independently and as part of a team
Compassionate patient-centered approach
