Roles & Responsibilities

• Provide patient-centered physical therapy evaluations and develop individualized treatment plans
• Implement therapeutic interventions to improve mobility, strength, balance, and function for veterans
• Document progress clearly and timely in the facility’s medical record system
• Collaborate with interdisciplinary team members to coordinate excellent patient care
• Educate patients and families on home programs, fall prevention, and safe mobility strategies
• Ensure equipment safety and recommend assistive devices as needed
• Maintain professional standards of practice and comply with facility policies

Qualifications

• Current and unrestricted Physical Therapist license in Montana, or eligibility to obtain one promptly
• Graduation from a CAPTE-accredited Physical Therapy program (DPT preferred)
• Experience working with geriatric or long-term care populations is preferred
• Strong documentation skills and familiarity with electronic medical records
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Excellent communication skills and a team-oriented mindset

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent accepted

Knowledge & Skills

• Clinical expertise in mobility, transfers, gait training, balance, and strength training
• Knowledge of geriatric conditions and common comorbidities in veteran populations
• Skilled in patient assessment, goal setting, and measurable outcome tracking
• Ability to work independently in a long-term care environment
• Strong interpersonal skills for working with patients, families, and care teams
