Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans to restore mobility and independence
• Provide hands-on and evidence-based interventions including therapeutic exercise and manual therapy
• Collaborate with nurses, physicians, and rehab team members to coordinate safe patient care
• Document progress clearly and timely in the facility EMR to support continuity of care
• Educate patients and families on home exercise programs and safe functional strategies
• Discharge planning and care transition coordination to ensure successful next steps

Qualifications

• Licensed Physical Therapist in Pennsylvania or able to obtain state licensure
• Doctor of Physical Therapy (DPT) preferred; related clinical degree acceptable with appropriate licensure
• Active BLS/CPR certification or willingness to obtain upon hire
• Previous experience in inpatient rehab, skilled nursing, or related setting preferred
• Strong documentation skills and familiarity with common EMR systems
• Excellent communication and teamwork skills to thrive in a multidisciplinary setting

Education

• Doctor of Physical Therapy (DPT) preferred
• Bachelor's or Master's in Physical Therapy acceptable with valid licensure

Knowledge & Skills

• Skilled in therapeutic exercise prescription and progression
• Familiar with gait training, transfers, and ADL training
• Competent in manual therapy techniques and modalities
• Ability to perform functional assessments and write measurable goals
• Strong clinical reasoning and patient education skills
• Comfortable working in fast-paced rehab and skilled nursing environments
