Roles & Responsibilities

• Assess, plan and implement nursing care for residents under RN supervision
• Provide continuity of care through direct patient care and clear documentation
• Administer medications and complete medication passes with competency
• Supervise CNAs and support staff to ensure safe, high-quality care
• Cross-train in other nursing departments as needed to support the team
• Communicate professionally and compassionately with residents, families and staff
• Accurately document care in the electronic medical record (EMR)

Qualifications

• Current California LVN license (required)
• Long-term care / Skilled Nursing Facility (SNF) experience required (Sub-Acute does not count)
• Must pass nursing medication test and demonstrate medication pass competency
• Current Basic Life Support (BLS) certification required
• Experience supervising CNAs and directing support staff
• Effective English verbal and written communication skills
• Basic computer skills and comfort using EMR systems (preferred)

Education

• Current California LVN license
• Graduate of an accredited Vocational Nursing/LVN program (typical requirement for licensure)

Knowledge & Skills

• Medication administration and medication pass procedures
• Skilled nursing care standards and long-term care protocols
• Electronic Medical Records (EMR) navigation and documentation
• Basic life support (BLS) knowledge and emergency response
• Interpersonal skills for professional interaction with residents, families and agencies
• Ability to supervise and mentor CNAs and support staff
