Roles & Responsibilities
- Provide compassionate, patient-centered care with attention to detail
- Administer medications and treatments accurately and safely
- Perform thorough assessments and report changes in condition
- Document care and maintain accurate patient records in the charting system
- Collaborate with nurses and interdisciplinary team members to deliver coordinated care
- Assist with activities of daily living and promote patient comfort and dignity
- Adhere to infection control and safety protocols to protect patients and staff
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Completion of an accredited Practical Nursing program
- Strong clinical judgment, good communication skills, and a team-first attitude
- Previous experience in long-term care, rehab, or acute care preferred but not required
Education
- Diploma or certificate from an accredited Practical Nursing program (LPN)
Knowledge & Skills
- Medication administration and documentation best practices
- Basic patient assessment and vital signs monitoring
- Familiarity with electronic health records and charting
- Infection prevention and control measures
- Effective patient and family communication
- Time management and prioritization in a fast-paced setting
