Shift Type: 12-hour shift (Total hours per shift: 12). Exact start/end times and days of week to be confirmed with the facility; shift type to be confirmed with hiring manager.
Hours per Week: 36
Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Provide compassionate direct patient care and monitor resident condition
- Administer medications and treatments safely and accurately
- Collaborate with the nursing team to develop and implement care plans
- Document patient status, care provided, and any changes in condition
- Communicate effectively with patients, families, and interdisciplinary staff
- Assist with admissions, transfers, and discharges as needed
- Support infection control practices and maintain a safe care environment
Qualifications
- Valid LPN license in Pennsylvania or eligibility to obtain one
- Current BLS/CPR certification (or ability to obtain upon hire)
- Demonstrated competence in medication administration and basic nursing procedures
- Strong communication skills and ability to work as part of a team
- Reliable, professional attitude with attention to patient safety and documentation
Education
- LPN diploma, practical nursing certificate, or equivalent
- High school diploma or GED
Knowledge & Skills
- Foundational nursing assessment and vital signs monitoring
- Medication administration and basic IV or wound care familiarity
- Electronic medical record (EMR) charting and documentation
- Infection control and standard precautions
- Time management and prioritization in a fast-paced care setting
- Patient-centered communication and compassionate care
