Roles & Responsibilities

• Provide compassionate bedside care and assist residents with activities of daily living
• Administer medications and treatments accurately and document in the medical record
• Monitor and record vital signs, reporting changes to the RN or provider promptly
• Collaborate with the care team to create and follow individualized care plans
• Perform timely charting and maintain accurate, compliant documentation
• Support a safe, clean environment and follow infection-control protocols
• Communicate clearly with residents and families to provide education and emotional support

Qualifications

• Valid LPN license in Pennsylvania (required)
• Completion of an accredited LPN program
• Current BLS/CPR certification (or willingness to obtain)
• Ability to pass background check and drug screen as required by facility
• Compassionate patient-centered approach with strong communication skills
• Previous experience in long-term care or similar setting is a plus

Education

• Graduate of accredited LPN (Practical Nursing) program
• High school diploma or GED

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable with charting and electronic medical records
• Strong observational skills for reporting changes in condition
• Teamwork-oriented with the ability to work independently when needed
• Good time management and prioritization during busy 12-hour shifts
• Familiarity with infection control and safety protocols
