Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents and patients
- Perform timely medication administration and follow medication safety protocols
- Assess patient status, monitor vital signs, and report changes to the care team
- Document care accurately and thoroughly in the patient chart/EHR
- Assist with wound care, basic treatments, and implementation of care plans
- Collaborate with nurses, CNAs, and interdisciplinary team members to ensure excellent outcomes
Qualifications
- Active LPN license (Pennsylvania) — required
- Completion of an accredited LPN program
- Current CPR/BLS certification (or willingness to obtain)
- Strong communication skills and a team-first attitude
- Previous experience in long-term care or similar setting preferred
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and medication safety
- Basic wound care and dressing changes
- Patient assessment and recognition of clinical changes
- Accurate charting and familiarity with electronic health records
- Infection control, safe patient handling, and documentation best practices
- Professional bedside manner and effective communication with patients and families
