Roles & Responsibilities
- Provide compassionate direct patient care and support daily living activities
- Administer medications and treatments accurately and on schedule
- Perform patient assessments and report changes promptly to the care team
- Document patient care and observations clearly in the medical record
- Collaborate with nurses, CNAs, and interdisciplinary staff to deliver coordinated care
- Assist with admissions, discharges, and transfers to ensure smooth transitions
- Maintain a safe, clean environment and follow infection control practices
Qualifications
- Current, active LPN license (able to practice in Pennsylvania preferred)
- BLS/CPR certification or willingness to obtain upon hire
- Completion of an accredited LPN program or vocational nursing certificate
- Comfortable administering medications, performing assessments, and documenting care
- Strong interpersonal and communication skills; patient-focused attitude
- Previous experience in long-term care or skilled nursing is a plus
Education
- LPN diploma or vocational nursing certificate
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic clinical assessments
- Charting and electronic health record familiarity
- Infection control and safe patient handling practices
- Time management and prioritization in a busy care setting
- Teamwork and clear communication with patients and staff
- Ability to follow protocols and escalate concerns appropriately
