Roles & Responsibilities

• Provide safe, compassionate direct patient care as a licensed practical nurse
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status and report changes to the care team promptly
• Collaborate with nurses, physicians, and support staff to deliver coordinated care
• Assist with admissions, discharges, and patient education to support recovery
• Maintain accurate patient records and uphold facility policies and procedures
• Support a positive, patient-centered environment by demonstrating professionalism and empathy

Qualifications

• Current LPN license in Pennsylvania required
• Basic Life Support (BLS) certification required or obtained upon hire
• Current immunizations and ability to meet facility health requirements
• Comfortable administering medications and performing clinical skills for adults
• Strong communication skills and ability to work well within a multidisciplinary team
• Ability to pass background check and any required screenings

Education

• Practical Nursing diploma or equivalent required

Knowledge & Skills

• Knowledge of fundamental nursing principles and nursing documentation
• Familiarity with medication administration and safety protocols
• Ability to assess patients and recognize changes in condition
• Basic clinical skills including wound care, vital signs, and ADL support
• Good time management and prioritization in a fast paced environment
• Comfort using electronic health record systems for charting
