Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to assigned residents
- Administer medications and treatments accurately and document thoroughly
- Monitor and report changes in patient condition promptly to the care team
- Collaborate with nurses, CNAs, and interdisciplinary staff to deliver coordinated care
- Maintain accurate, timely charting in the resident record
- Educate residents and families about care plans and health needs
- Follow facility policies, infection control standards, and safety protocols
Qualifications
- Current LPN license in Pennsylvania (required)
- Current CPR/BLS certification (required or obtain upon hire)
- Demonstrated clinical competence and strong patient care skills
- Comfortable with medication administration and basic nursing procedures
- Ability to pass background check, drug screen, and meet facility onboarding requirements
- Previous experience in long-term care or related setting preferred
Education
- LPN diploma or equivalent (required)
Knowledge & Skills
- Strong patient assessment and clinical judgment
- Medication administration and documentation
- Familiarity with infection control and safety practices
- Effective communication and compassion with patients and families
- Basic proficiency with electronic health records and charting
- Time management and ability to prioritize in a busy environment
- Team player who collaborates well with interdisciplinary staff
