Roles & Responsibilities

Provide high-quality nursing care to clinic patients within scope of LPN licensure
Take and record vital signs, assess patient status, and escalate concerns to providers
Triage patient phone calls and coordinate timely follow-up
Process medication refills and manage medication-related documentation
Complete prior authorizations to help patients receive prescribed therapies
Act as primary liaison between patients, clinical staff, and providers to ensure smooth care delivery
Give direction to CNAs and schedulers to support efficient clinic workflow
Collaborate with interdisciplinary team members to meet patient needs and improve outcomes

Qualifications

Graduate of an accredited Nursing Program
Current State LPN license
BLS (Basic Life Support) certification
Comfortable performing triage, taking vital signs, and managing medication refills
Ability to complete prior authorizations and communicate effectively with providers and staff
Experience working in clinic nursing and collaborating with interdisciplinary teams (preferred)

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Patient care and basic clinical assessment
Vital signs measurement and interpretation
Telephone triage and patient communication
Medication management and refill processing
Prior authorization processes and documentation
CNA supervision and scheduler coordination
Clinic workflow and interdisciplinary collaboration
Familiarity with gastroenterology clinic procedures (preferred)
