Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients in a long-term care setting
• Administer medications and treatments accurately and safely according to orders
• Monitor and document patient condition, vital signs, and response to care
• Collaborate with the interdisciplinary care team to implement and update care plans
• Assist residents with activities of daily living while preserving dignity and independence
• Communicate effectively with families and team members about changes in condition
• Maintain accurate electronic and paper documentation in a timely manner
• Adhere to infection control, safety, and facility policies at all times

Qualifications

• Active Licensed Practical Nurse (LPN) credential in the state of practice
• Current Basic Life Support (BLS) or CPR certification (recommended/typical requirement)
• Experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong clinical assessment, medication administration, and documentation skills
• Ability to work independently and as part of a collaborative care team

Education

• Graduate of an accredited Practical/Vocational Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Skilled in medication administration and infection control practices
• Proficient with resident care, ADLs, and chronic condition management
• Comfortable with charting and documentation (electronic health records experience a plus)
• Excellent communication, time management, and prioritization skills
• Able to follow established protocols and contribute to a positive care environment
