Roles & Responsibilities

• Deliver compassionate hands-on nursing care to residents within your scope as an LPN
• Administer medications and treatments safely and accurately, following facility policies
• Monitor patients, observe changes in condition, and report findings to the care team
• Maintain clear, timely documentation in the patient chart and electronic record
• Collaborate with RNs, physicians and other team members to create and follow care plans
• Provide patient and family education with patience and empathy
• Support safe admissions, transfers and discharges to ensure continuity of care
• Promote a safe, clean environment and follow infection control best practices

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) or CPR certification (required or to be obtained promptly)
• Comfort with medication administration, wound care and routine assessments
• Strong communication skills and a team-oriented attitude
• Previous long-term care or skilled nursing experience is a plus

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic clinical assessment and vital sign monitoring
• Electronic health record (EHR) navigation and accurate charting
• Infection prevention and control procedures
• Time management and prioritization during busy shifts
• Empathy, patient advocacy and clear interpersonal communication
