Roles & Responsibilities

• Provide compassionate bedside care and daily support to residents and patients
• Administer medications and treatments accurately following facility protocols
• Perform timely assessments and communicate changes in condition to the care team
• Document care, observations, and interventions clearly and professionally
• Assist with activities of daily living while promoting patient dignity and independence
• Collaborate with nurses and interdisciplinary staff to deliver coordinated care
• Follow infection control standards and safety policies to protect patients and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (PA licensure expected for Smethport assignments)
• Current CPR/BLS certification or willingness to obtain upon hire
• Comfort with 12-hour clinical shifts and adaptable scheduling
• Demonstrated ability to provide safe, patient-centered care and good clinical judgement
• Willingness to complete background check, drug screen, and required health screenings

Education

• Completion of an accredited practical nursing program and active LPN licensure

Knowledge & Skills

• Strong medication administration and documentation skills
• Basic assessment skills and familiarity with common long-term care conditions
• Wound care, catheter care, and ADL assistance experience
• Proficient communication and teamwork in a clinical setting
• Comfortable with electronic charting systems and accurate record keeping
• Time management and prioritization during busy 12-hour shifts
