Licensed Practical Nurse - Skilled Nursing
Location: Smethport, PA
Start Date: 04-02-2026
Contract Type: Per-Diem
Hours per Week: 40
Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Provide compassionate, high-quality bedside care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document per policy
- Monitor resident condition, report changes promptly, and collaborate with the care team
- Assist with activities of daily living while promoting dignity and independence
- Maintain clear, timely, and accurate records in the resident charting system
- Support a positive environment by mentoring CNAs and communicating with families
- Follow infection control and safety protocols to protect residents and staff
Qualifications
- Active LPN license in Pennsylvania or compact/state eligibility as required
- Graduation from an accredited LPN/Vocational Nursing program
- Current CPR/BLS certification (or willingness to obtain upon hire)
- Successful background check and health screening per facility policy
- Strong communication skills and a team-oriented approach
- Comfortable with medication administration, charting, and basic wound care
Education
- Graduation from an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Solid knowledge of nursing fundamentals and medication administration
- Skilled in resident assessment, documentation, and care planning
- Familiarity with infection control and clinical safety practices
- Ability to prioritize tasks in a fast-paced long-term care environment
- Good interpersonal skills for working with residents, families, and interdisciplinary teams
