Roles & Responsibilities

• Provide compassionate nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately according to physician orders
• Monitor resident conditions, document observations, and communicate changes to the care team
• Assist with activities of daily living while encouraging resident independence
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Maintain accurate and timely clinical documentation and care plans
• Support a safe, clean environment and adhere to infection control practices

Qualifications

• Active LPN license in Pennsylvania (or compact/state-appropriate) required
• Completion of an accredited practical nursing program or vocational certification
• Current BLS/CPR certification or ability to obtain upon hire
• Comfortable working in a long-term care or skilled nursing environment; previous LTC experience preferred
• Strong communication skills and the ability to collaborate with interdisciplinary teams
• Excellent attention to detail and a resident-centered approach

Education

• Practical Nursing diploma or certificate from an accredited program
• LPN licensure required

Knowledge & Skills

• Medication administration and basic nursing assessments
• Wound care basics and monitoring for clinical changes
• Charting and electronic medical record familiarity
• Understanding of infection prevention and safety protocols
• Ability to prioritize tasks in a fast-paced care setting
• Compassionate bedside manner and strong interpersonal skills
