Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, ensuring dignity and safety
• Administer medications and treatments accurately and document per facility policy
• Perform thorough patient assessments and communicate changes to the care team
• Collaborate with nurses, therapists, and physicians to develop and follow individualized care plans
• Monitor and record vital signs, intake/output, and clinical observations
• Support admissions, transfers, and discharges with smooth, patient-centered coordination
• Promote infection control practices and maintain a clean, safe environment
• Mentor and support CNAs and other staff to foster a positive team culture

Qualifications

• Active LPN license in Massachusetts (for LPN applicants) or active RN license in Massachusetts (for RN applicants)
• Current Basic Life Support (BLS) certification required
• Previous experience in long-term care, skilled nursing, or rehab settings preferred
• Strong communication skills and ability to work well in an interdisciplinary team
• Demonstrated ability to document accurately and follow policies/procedures
• Reliable, compassionate, and resident-focused approach to care

Education

• Graduate of an accredited LPN or RN nursing program (diploma, associate degree, or equivalent)
• Additional certifications or continuing education in geriatric/rehab nursing a plus

Knowledge & Skills

• Skilled in medication administration and basic clinical procedures
• Proficient with patient assessment and clinical observation
• Knowledge of infection prevention and wound care principles
• Competent in electronic health record charting and documentation
• Strong prioritization, time management, and critical-thinking skills
• Excellent interpersonal skills for working with residents, families, and team members
