Roles & Responsibilities

• Provide compassionate, high-quality direct patient care consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Monitor patients' conditions, report changes to the care team, and assist with care plans
• Collaborate with RNs, physicians, and therapy staff to ensure coordinated care
• Perform routine assessments, wound care, and support activities of daily living
• Maintain accurate, timely charting in the patient record and follow facility protocols
• Support a safe, respectful environment for patients and families

Qualifications

• Current, active Licensed Practical Nurse (LPN) license for Pennsylvania
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Comfortable providing hands-on care in a long-term care / skilled nursing environment
• Strong communication skills and ability to work effectively with a multidisciplinary team
• Professional attitude, dependability, and a patient-first mindset

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)
• Active PA LPN licensure

Knowledge & Skills

• Clinical skills in medication administration and routine nursing assessments
• Familiarity with common long-term care procedures and protocols
• Ability to document accurately in electronic or paper medical records
• Strong interpersonal skills for working with patients, families, and staff
• Time management and prioritization in a dynamic care environment
