Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments safely and accurately
• Assess and document patient condition, reporting changes to the care team
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Support resident comfort, dignity and independence through daily care activities
• Participate in shift handoffs and maintain accurate, timely charting
• Help maintain a safe, clean environment and adhere to infection control protocols

Qualifications

• Current, active LPN license in the state of Pennsylvania (required)
• Basic Life Support (BLS) / CPR certification (required or obtained upon hire)
• Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
• Ability to pass background check and drug screening
• Strong communication skills and a collaborative mindset
• Dependable, punctual, and able to follow established policies and procedures

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Clinical competency in medication administration and basic nursing procedures
• Knowledge of infection control and resident safety practices
• Comfortable with electronic or paper charting systems
• Ability to prioritize tasks in a fast-paced care environment
• Empathy, patience, and strong interpersonal skills with residents and families
