Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and on time
- Monitor and record vital signs, reporting changes promptly
- Perform focused patient assessments and communicate findings to the care team
- Assist patients with activities of daily living while promoting independence
- Document care thoroughly in the medical record to ensure continuity
- Collaborate with nurses, physicians, and support staff to deliver coordinated care
- Support a safe, clean environment and follow infection control protocols
Qualifications
- Valid Licensed Practical Nurse (LPN) license (PA license required or eligible)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Strong clinical skills in medication administration and basic nursing procedures
- Effective communication skills and a patient-centered approach
- Preference for previous LPN experience in long-term care or similar settings
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Knowledge of medication administration and documentation best practices
- Skill with patient assessment, vital sign monitoring, and basic wound care
- Familiarity with electronic health records and accurate charting
- Ability to prioritize tasks and manage time in a fast-paced care environment
- Strong interpersonal skills for working with patients, families, and team members
