Roles & Responsibilities

• Provide compassionate, patient-centered care that improves daily living
• Administer medications and treatments safely and in line with policy
• Perform routine assessments and promptly report changes in condition
• Document care accurately and maintain clear electronic and paper records
• Support activities of daily living and encourage patient independence
• Collaborate with nurses, therapists, and interdisciplinary team members to deliver coordinated care
• Assist with admissions, transfers, and discharges to ensure continuity of care

Qualifications

• Active LPN license in Pennsylvania or eligibility to obtain licensure
• Completion of an accredited LPN/licensed practical nurse program
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Comfort with medication administration and basic nursing procedures
• Strong communication skills and a teamwork-oriented attitude
• Previous long-term care or acute care experience preferred but not required

Education

• Graduate of an accredited LPN/licensed practical nurse program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital sign monitoring
• Infection control and safety procedures
• Familiarity with electronic health records and charting
• Time management and prioritization in a fast-paced setting
• Empathy, patient advocacy, and strong interpersonal skills
