Roles & Responsibilities
- Provide compassionate direct nursing care to residents, ensuring dignity and comfort
- Administer medications and treatments accurately and on schedule
- Perform thorough assessments and report changes in condition promptly
- Document care, observations, and interventions clearly in the medical record
- Collaborate with the interdisciplinary team to develop and follow care plans
- Assist with activities of daily living and promote resident independence
- Maintain a safe, clean environment and follow infection control practices
- Communicate effectively with residents and families to support great care experiences
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (preferred or required per facility policy)
- Comfortable working in a long-term care or skilled nursing environment
- Strong communication and teamwork skills
- Previous LPN experience preferred but enthusiastic new grads are welcome to apply
Education
- Graduate of an accredited Practical Nursing program or equivalent diploma/certificate
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Able to perform resident assessments and recognize changes in condition
- Familiarity with electronic medical records and accurate charting
- Knowledge of infection prevention and safety protocols
- Good time management and the ability to prioritize resident care
