Roles & Responsibilities

• Provide compassionate direct patient care, administering medications and treatments per plan
• Monitor and document patient conditions, reporting changes to the care team promptly
• Support nursing assessments and implement care plans with attention to detail
• Collaborate with RNs, providers, and support staff to ensure safe, coordinated care
• Maintain accurate, timely charting in the patient record system
• Assist with patient education and discharge instructions in a clear, empathetic way
• Follow infection control and safety protocols to protect patients and staff

Qualifications

• Current LPN license (or eligible) — ready to practice in Pennsylvania
• Completion of an accredited Practical Nursing program
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Current immunizations and ability to pass required health screenings
• Reliable, professional demeanor with strong communication and teamwork skills
• Willingness to complete background checks and facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Medication administration and accurate documentation
• Basic patient assessment and vital signs monitoring
• Familiarity with electronic health records and charting best practices
• Infection prevention, safe patient handling, and standard precautions
• Strong interpersonal skills and ability to communicate with patients and families
• Ability to prioritize tasks in a fast-paced clinical environment
