Roles & Responsibilities

• Deliver compassionate, high-quality direct patient care to assigned residents and patients
• Administer medications and treatments accurately and document all interventions
• Perform thorough assessments and report changes in patient status promptly
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate patient care
• Maintain clear, accurate charting in the electronic medical record
• Adhere to infection control standards and safety protocols to protect patients and staff
• Support patient and family education with empathy and clear communication

Qualifications

• Current, active LPN license (practical nurse) — Georgia license preferred
• Graduation from an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Ability to pass required background checks and facility onboarding
• Strong communication skills and a team-oriented attitude
• Prior experience in acute care, long-term care, or hospital settings is a plus

Education

• Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Skilled in patient assessment, wound care basics, and vital sign monitoring
• Comfortable using electronic medical records and clinical documentation
• Understanding of infection prevention and standard precautions
• Good time management, prioritization, and teamwork skills
