Roles & Responsibilities

• Provide direct patient care with compassion and clinical competence, delivering safe and effective bedside nursing
• Administer medications and treatments according to physician orders and facility policies
• Monitor and document patient condition, changes, and interventions clearly in the medical record
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Support residents and families with respectful communication and patient-centered care
• Participate in shift handoffs and reporting to ensure continuity and quality of care

Qualifications

• Active LPN license (practical nurse) — state licensure required; Pennsylvania licensure expected for this location
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable with medication administration, wound care basics, and routine clinical procedures
• Strong communication skills and ability to work well in a team-based, resident-focused environment
• Ability to pass required background checks and health screenings

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and vital sign monitoring
• Familiarity with electronic medical records (EMR) or willingness to learn
• Time management and prioritization in a fast-paced care setting
• Patient advocacy and family-centered communication
