Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living
• Assist patients with mobility, transfers and safe positioning
• Monitor and record vital signs and report changes promptly to nursing staff
• Maintain accurate, timely documentation in the patient record
• Support the team with timely communication and follow-through on care plans
• Help maintain a clean, safe environment and follow infection-control practices
• Assist with meal support and basic patient comfort needs

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (state of New Hampshire preferred)
• Travel-capable LNA (T-LNA) or willingness/ability to obtain traveler credentials as required
• Ability to work the mix of 12-hour and 8-hour day shifts and commit to a 13-week assignment
• Strong communication skills and a team-first mindset
• Recent clinical experience in long-term care, assisted living, or similar settings preferred

Education

• High school diploma or equivalent
• Completion of an approved nursing assistant training program

Knowledge & Skills

• Skilled in patient care basics (ADLs, bathing, feeding, toileting)
• Comfortable taking and reporting vital signs and observing patient condition
• Knowledge of safe transfer and mobility techniques
• Basic charting and documentation skills
• Good infection control and safety awareness
• Team collaboration and clear communication with nurses and interdisciplinary staff
