Roles & Responsibilities

• Perform comprehensive patient evaluations and develop individualized Plans of Care to improve outcomes
• Deliver skilled nursing care during home visits and adapt interventions as patient needs change
• Document clinical findings and progress notes clearly and promptly to ensure continuity of care
• Coordinate services and communicate condition changes with physicians and the interdisciplinary team
• Provide compassionate patient and family counseling to support comfort and informed decision making
• Teach and mentor fellow nursing staff and contribute to in-service education to elevate care standards
• Initiate preventive and rehabilitative nursing procedures to maximize patient independence and quality of life
• Support quality improvement and patient safety initiatives through clear communication and best practices

Qualifications

• Active Registered Nurse (RN) license in Colorado required
• Graduate of an accredited nursing program: ASN/ADN or BSN required; BSN preferred
• BLS (Basic Life Support) certification required; hospice and home health certifications are a plus
• Valid driver license and reliable transportation for home visits
• Home health or hospice nursing experience preferred; minimum 1 year recommended
• Strong clinical judgment, excellent documentation skills, and effective interdisciplinary communication

Education

• ASN/ADN or BSN required; BSN preferred

Knowledge & Skills

• Proficient with the nursing process: assessment, planning, implementation, evaluation
• Proven patient care and skilled nursing ability in home health settings
• Experience with case management and Plan of Care development
• Clear clinical documentation and timely progress notes
• Excellent coordination of services and physician communication
• Patient and family counseling techniques delivered with compassion
• Teamwork, teaching, and mentoring to support peers and elevate care
• Commitment to quality improvement and patient safety best practices
