Roles & Responsibilities

Provide comprehensive outpatient family medicine care across the lifespan with compassion and clinical excellence
Build and maintain long-term relationships with patients and their families, focusing on continuity of care
Collaborate with an experienced clinical and administrative team to coordinate patient-centered treatment plans
Manage patient panels, perform preventive care, chronic disease management, and acute care visits
Participate in quality improvement initiatives to enhance care delivery in a rural community setting

Qualifications

MD or DO from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State (must obtain NY license)
DEA certification or eligibility
Passion for community-focused, patient-centered primary care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Outpatient primary care experience and comfort managing patients across the lifespan
Strong clinical judgment and ability to provide longitudinal care
Excellent communication skills and a collaborative approach to working with clinical staff
Experience or interest in rural/community health is a strong plus
Commitment to providing patient-centered, high-quality care
