Roles & Responsibilities

• Deliver skilled occupational therapy interventions as a COTA in a skilled nursing facility, focusing on resident independence and safety
• Document therapy sessions and outcomes accurately in NetHealth and PCC EMR to support continuity of care
• Collaborate with therapists, nursing staff, and interdisciplinary team members to create and implement person-centered care plans
• Meet productivity expectations (90%) while maintaining high-quality, empathetic patient care
• Follow Continuum Rehabilitation Group (CRG) policies and procedures and facility-specific protocols
• Be present on-site and punctual to ensure guaranteed hours and consistent resident coverage
• Maintain professional appearance (black scrubs) and represent the facility positively to residents and families

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) credential
• 1-3 years of experience, SNF/long-term care experience preferred
• Comfortable working full-time on-site and meeting productivity requirements
• Familiarity with NetHealth and PCC EMR systems or ability to learn quickly
• Ability to follow company policies (CRG P & P) and facility procedures
• Professionalism, strong communication skills, and teamwork mindset

Education

• Certified Occupational Therapy Assistant (COTA)

Knowledge & Skills

• Skilled in ADL training, therapeutic activities, and functional mobility support
• Proficient or trainable in NetHealth and PCC electronic medical records
• Excellent documentation practices and outcome measurement
• Strong time management to balance caseload and productivity targets
• Ability to communicate effectively with residents, families, and interdisciplinary teams
• Understanding of SNF care environment and skilled mix dynamics
