Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Assist with transfers, mobility, ambulation and safe patient handling
• Support rehabilitation plans by following care team instructions and documenting progress
• Monitor patients for changes in condition and promptly report concerns to nursing staff
• Maintain a clean, safe environment and adhere to infection control practices
• Communicate clearly with patients, families and the interdisciplinary team to promote positive outcomes

Qualifications

• Current Certified Nursing Assistant (CNA) credential required
• T-CNA (trainee CNA) designation accepted per facility classification
• Relevant clinical experience in long-term care or rehabilitation preferred
• Active CPR/BLS certification preferred or willing to obtain
• Ability to pass facility onboarding, background check and health screenings

Education

• High school diploma or equivalent and completion of an approved CNA program
• Current CNA certification as required by state regulations

Knowledge & Skills

• Solid knowledge of patient transfer and ambulation techniques
• Comfortable with basic clinical documentation and observations
• Strong interpersonal and communication skills with patients and care teams
• Focus on safety, infection control and patient dignity
• Ability to follow care plans and adapt to a rehab-focused environment
