Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents according to care plans
• Administer medications and treatments accurately, documenting each intervention
• Monitor patient status and report changes to the interdisciplinary team promptly
• Collaborate with RNs, therapists, and support staff to deliver person-centered care
• Educate residents and families on care plans, medications, and safety measures
• Maintain accurate clinical documentation and comply with facility policies
• Assist with admissions, transfers, and discharge planning as needed

Qualifications

• Current, active LPN license (Licensed Practical Nurse) — ability to practice in New Mexico preferred
• Graduate of an accredited Practical Nursing program or equivalent
• Basic Life Support (BLS) certification required; ACLS/PALS as applicable preferred
• Clinical experience in long-term care, medical-surgical, or similar settings preferred
• Ability to pass required health screenings and background checks
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing (LPN) program or vocational nursing diploma

Knowledge & Skills

• Solid medication administration and documentation skills
• Ability to assess and report changes in patient condition
• Familiarity with long-term care routines, wound care, and ADL support
• Good time management and prioritization during busy shifts
• Comfort with electronic health record systems and basic computer use
• Effective interpersonal skills for working with residents, families, and staff
