Roles & Responsibilities

• Provide compassionate bedside care to residents, following care plans and facility protocols
• Administer medications and treatments accurately and safely
• Perform thorough assessments and monitor patients for changes in condition
• Document care clearly and promptly in the electronic medical record
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Support admissions, discharges and safe transfers to ensure continuity of care
• Educate residents and families on care plans and safety measures
• Contribute to a positive team culture by mentoring peers and sharing best practices

Qualifications

• Active Licensed Practical Nurse (LPN) credential or ability to obtain New Mexico state licensure prior to start
• Current Basic Life Support (BLS) certification or ability to obtain before starting
• Experience working in long term care, skilled nursing or rehab settings preferred
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork and documentation abilities
• Ability to work the assigned block schedule (three 12-hour shifts and one 4-hour shift, Monday-Thursday)

Education

• Completion of an accredited practical nursing program or diploma

Knowledge & Skills

• Medication administration and IV/IM basics where applicable
• Wound care and basic restorative nursing skills
• Familiarity with electronic medical records and clear clinical documentation
• Patient safety, infection control and fall prevention practices
• Time management and prioritization during busy 12-hour shifts
• Compassionate patient and family communication
