Roles & Responsibilities

• Provide compassionate hands-on nursing care for nursing home residents
• Administer medications and treatments accurately and document thoroughly
• Assist residents with activities of daily living while promoting independence
• Observe and report changes in resident condition to the care team
• Collaborate with RNs, CNAs, therapists, and families to create person-centered care plans
• Maintain clear, timely clinical documentation in resident records
• Follow infection control practices and facility policies to keep residents safe
• Support a positive environment that promotes dignity and quality of life

Qualifications

• Current Licensed Practical Nurse (LPN) license — must be valid for New York
• CPR/BLS certification required or willingness to obtain promptly
• Comfortable working in a long-term care / nursing home environment
• Strong communication skills and a collaborative approach to care
• Ability to pass facility background checks and required health screenings
• Previous experience in geriatric or long-term care settings is a plus

Education

• LPN diploma or vocational nursing certificate
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and skin integrity monitoring
• Skilled in resident assessments and reporting changes
• Familiarity with electronic medical records and charting
• Knowledge of infection control and elder care standards
• Good time management and ability to prioritize resident needs
• Compassionate interpersonal skills for residents and families
