Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled rehab and nursing setting
• Administer medications and treatments accurately and document in the patient record
• Perform thorough head-to-toe assessments and report changes promptly to the care team
• Collaborate with therapists, physicians and support staff to deliver individualized care plans
• Support residents through recovery milestones with encouragement and clinical expertise
• Maintain clear, timely documentation and ensure regulatory compliance
• Participate in shift handoffs and contribute to a positive, team-first culture

Qualifications

• Active LPN or RN license (Massachusetts) — current and in good standing
• Current BLS/CPR certification
• Experience in long-term care, rehab, or skilled nursing preferred
• Strong communication and teamwork skills with a resident-centered mindset
• Ability to pass standard background checks, TB screening and immunization requirements

Education

• LPN: Completion of an accredited practical nursing program
• RN: Diploma, ADN or BSN from an accredited nursing program (if applying as RN)

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable performing resident assessments and wound care basics
• Proficient in clinical documentation and following care plans
• Good time management and ability to prioritize in a busy setting
• Team collaboration, clear communication, and advocacy for residents
