Roles & Responsibilities
- Evaluate patients and develop individualized occupational therapy treatment plans
- Assist patients in improving activities of daily living (ADLs) and functional independence
- Collaborate with an interdisciplinary rehab team to support optimal patient outcomes
- Educate patients and caregivers on adaptive techniques, safety, and exercises
- Maintain accurate and timely documentation to support quality care
Qualifications
- Active California Occupational Therapist (OT) license required
- SNF or rehab experience preferred
- Strong clinical judgment and effective communication skills
- Ability to work independently and as part of a collaborative team
Knowledge & Skills
- Occupational Therapy principles and interventions
- Clinical judgment applied to patient evaluation and treatment planning
- Clear communication with patients, caregivers, and rehab team members
- Teamwork within an interdisciplinary skilled nursing facility setting
- Accurate clinical documentation and charting
